What You'll Need When Applying

Our grant is brief and easy to complete, but you’ll need some information handy to complete the application, including: 

• Details of brain tumor diagnosis, including date, treating hospital and treating doctors.

• Information about dependents.

• Details of employment status for yourself and your spouse/partner, including dates of hire and income.

• A statement describing your request for assistance, including the type, amount needed and duration.

To Protect Its Interests, The Michael Matters Foundation Requires:

• A copy of Applicant’s pathology report.

• Copy of Applicant household federal tax return for the most recent year.

• A copy of Applicant driver’s license or State ID
